Healthy Children &
Youth Program

 

The Healthy Children and Youth (HCY) program is designed to help secure care for disabled youth across Missouri. It provides service coordination and authorization for services deemed medically necessary for people under 21 years of age.

This includes home visits and resources to ensure children can remain safely in their homes with their families.

Eligibility:

The participant must:

*   Be a Missouri resident

*   Be birth to age 21

*   Need medically necessary services

*   Be enrolled in MO HealthNet
